Dare Home Health and Hospice provide care and services to patients in their homes in order to meet their comprehensive medical, therapeutic, and supportive services needs. The home health program is designed to asstist patients in achieving their maximum level of independence through teaching and rehabilitative services.

Hospice provides palliative (comfort care) to terminally ill patients in their home and support their families throuh the accompanying grief and bereavment. An interdisciplinary approach to care ensures patient's neesd are addressed effetively and holistically.

Both programs are Medicare/Medicaid certified and accredited by the Accrediation Commission for Healthcare, INC.

Eligibility: Home Health services are available to homebound individuals, who are currently under the care of a qualifying physician and in need of skilled care. Hospice services are available to individulas with a terminal diagnosis, a limited life expectancy, who are under the care of a qualifying physician. Both programs serve all ages and economic groups. Services are made available regardless of ability to pay.

Fees: Home Health and Hospice accepts Medicare, Medicaid, private insurance, Veteran's Administration, Worker's Compensation and files insurance claims as a convenience.

Dare Respite Care is a faith based, volunteer program offering respite services for adult caregivers caring for frail, elderly, disabled or shut-in individuals at home. Services are designed to be periodic and episodic offering caregivers a brief respite from their daily care-giving role, while providing a community connection and socialization to the care recipient.

Eligibility: Determined on a case-by-case basis. Please call for an in-home evaluation.

Fees: This program is free of charge to anyone meeting eligibility requirements.
