Community Care Clinic of Dare (CCC of Dare)

A partnership between the Dare County Health Department, the Outer Banks Hospital, and the Albemarle Hospital Foundation, CCC of Dare strives to provide basic health care for people who do not have or cannot afford adequate health insurance. Services will include primary care, medications at low or little cost. Sites of service: North Beach Center in Kitty Hawk, Island Medical in Manteo, Health Department in Buxton. Call 252-261-3041 for current hours and for appointments.

Eligibility: Dare County Residents and individuals who work in Dare County ages 18 and up, who do not have medicare, medicaid or health insurance.

Fees: No fees - donations accepted

Appointments are required for some services and highly recommended for all services. Walk-ins may be accepted, but best to call ahead. Services offered include:

Diabetes Self-Management Training is for persons with diabetes and their families. Program includes: nursing assessment, nutritional assessment, and individual and group education.

Eligibility: A Physician, Nurse Practitioner, or Physician's Assistant referral is required

Fees: Accepts Medicare, Medicaid, private insurance, and private pay based on a sliding fee scale.

HCOB is a partnership working toward a healthier Dare County. The partnership is coordinated and facilitated by the Dare County Department of Public Health and consists of representatives from over 35 local agencies and organizations. HCOB conducts a comprehensive community health assessment every four years and brings concerned citizens together to address identified health & safety issues.

An overall healthy lifestyle is a major component in preventing disease and disease related complications. In addition to exercise, nutrition is an essential part of a healthy lifestyle. Choosing foods you enjoy and planning meals to fit your lifestyle is important. Medical Nutrition Therapy (MNT) is a service which provides individualized nutrition assessment and counseling. Following a physician referral the patient will receive: 1 hour evaluation with Registered Dietitian to assess individual needs; follow up appointment (s) 15-30 minutes each (The number of follow up appointments will be determined with each session). A physician referral is required.

Eligibility: A physician referral is required.

Fees: Services are available regardless of ability to pay. Cost is based on a sliding scale fee according to income. Most insurance will cover Medical Nutrition Therapy. Contact your insurance company to find out verification or pre-authorization requirements.

Offers a variety of activities including speakers on diabetes related topics, cooking demonstrations, and specific holiday activities. Stepping Stones is sponsored by Regional Medical Services in partnership with the Dare County Department of Public Health Diabetes Education Program. The group meets in September through May at Regional Medical Center, 5200 N. Croatan Hwy, Kitty Hawk. Anyone interested in learning about diabetes is welcome.
